Trinity Home Health Services Inc. ("Trinity," "we," "us," or "our") operates the Trinity CareView mobile application ("CareView" or "the App") and the supporting web platforms at trinitycareview.com and trinityemr.com (collectively, "the Services"). This Privacy Policy explains how we collect, use, store, and protect your personal and health information when you use our Services.

CareView is designed exclusively for patients enrolled in Trinity Home Health Services remote monitoring programs. By using the App, you agree to the practices described in this Privacy Policy.

1. Information We Collect

1.1 Personal Information

When you are enrolled in a Trinity remote monitoring program, we collect the following information to create and maintain your account:

• Full name — provided by your healthcare provider during enrollment
• Phone number — used for account authentication via SMS verification code
• Patient ID — assigned by your healthcare provider to link your account to your medical record

1.2 Health and Vital Signs Data

The App captures health data from compatible Bluetooth medical devices that you pair with your phone. This includes:

• Blood pressure readings — systolic pressure, diastolic pressure, and heart rate from iHealth blood pressure monitors
• Weight readings — body weight from iHealth smart scales
• Blood glucose readings — blood glucose levels from iHealth glucose meters
• Daily health check-in responses — answers to screening questions about symptoms such as headaches, vision changes, swelling, and other indicators provided by your care team
• Timestamps — the date and time each reading or check-in was recorded

1.3 Messages

If you use the in-app messaging feature to communicate with your care team, we collect the content of those messages along with your name, phone number, and the date and time the message was sent.

1.4 Device and Technical Data

We collect limited technical data necessary for the App to function:

• Paired device identifiers — the model and MAC address of Bluetooth medical devices you pair with the App, stored locally on your phone
• Device type and operating system version — used to ensure compatibility and troubleshoot issues

We do not collect your geographic location, contacts, browsing history, advertising identifiers, or any data unrelated to your remote health monitoring.

2. How We Use Your Information

We use your information solely to provide, maintain, and improve the remote monitoring services described above. We do not use your data for advertising, marketing, analytics, profiling, or any purpose unrelated to your healthcare.

3. How We Share Your Information

Your health data is shared only with the following parties and only for the purposes described:

• Your Trinity Health care team — your assigned healthcare providers who review your vital signs and health check-in data as part of your remote monitoring program. Data is transmitted to the Trinity CareView electronic medical record system hosted at trinityemr.com.
• SMS delivery provider — we use a third-party SMS service (ClickSend) to deliver one-time verification codes for account authentication and to forward in-app messages to your care team. ClickSend processes your phone number and message content solely for delivery purposes and does not retain or use this data for any other purpose.

We do not sell, rent, trade, or otherwise disclose your personal or health information to any third party for advertising, marketing, data brokerage, or any purpose unrelated to the delivery of your healthcare services.

4. Bluetooth and Device Communication

CareView uses Bluetooth Low Energy (BLE) to communicate with compatible medical devices. When you initiate a measurement:

• The App scans for nearby paired medical devices using Bluetooth
• A direct connection is established between your phone and the medical device
• Vital signs data is transferred from the device to the App
• The Bluetooth connection is closed after the reading is complete

Bluetooth is used exclusively for communication with compatible medical devices. The App does not use Bluetooth to collect location data, communicate with non-medical devices, or transmit data to any third party. Bluetooth permission is required for the App to function and cannot be used without it.

5. Data Storage and Security

5.1 Local Storage

Your vital signs readings and paired device information are stored locally on your phone in an encrypted database. This allows you to view your reading history and ensures readings are preserved if your phone temporarily loses internet connectivity. Locally stored readings are synchronized with your medical record when connectivity is restored.

5.2 Server Storage

When your readings are synchronized, they are transmitted using HTTPS encryption to secure servers operated by Trinity Home Health Services. Your medical record data is stored in the Trinity CareView EMR system at trinityemr.com. Access to this data is restricted to authorized Trinity healthcare personnel involved in your care.

5.3 Security Measures

We implement the following measures to protect your data:

• HTTPS/TLS encryption for all data transmitted between the App and our servers
• SMS-based authentication to verify your identity before granting access to your account
• Local database encryption on your device
• Role-based access controls limiting EMR access to authorized care team members
• Duplicate detection to prevent duplicate records from being created in your medical record

6. Data Retention

Your health data is retained in the Trinity CareView EMR system for as long as required by applicable federal and state healthcare record retention laws and Trinity's internal medical records retention policies. Local data stored on your phone persists until you uninstall the App or clear the App's data through your phone's settings.

7. Your Rights and Choices

You have the following rights regarding your data:

• Access — You can view your vital signs history and health check-in data directly within the App at any time.
• Deletion of local data — You can delete all locally stored data by uninstalling the App from your phone.
• Deletion of server data — To request deletion of your data from Trinity's servers, contact us using the information provided below. Please note that we may be required to retain certain records under applicable healthcare regulations.
• Withdraw from monitoring — You may stop using the App at any time. To formally withdraw from your remote monitoring program, contact your Trinity care team.
• Bluetooth permissions — You may revoke Bluetooth permission through your phone's settings at any time, though the App will be unable to capture readings from your medical devices without it.

8. California Privacy Rights

If you are a California resident, you may have additional rights under the California Consumer Privacy Act (CCPA), including the right to know what personal information we collect, the right to request deletion, and the right to opt out of the sale of personal information. We do not sell personal information. To exercise any rights under the CCPA, contact us using the information provided below.

9. HIPAA Notice

Health information collected through CareView may be considered Protected Health Information (PHI) under the Health Insurance Portability and Accountability Act (HIPAA). Trinity Home Health Services handles all PHI in accordance with applicable HIPAA requirements. For complete details on how your health information is used and disclosed, please refer to the Trinity Home Health Services Notice of Privacy Practices, which is provided to you upon enrollment.

10. Children's Privacy

CareView is intended for use by patients enrolled in Trinity Home Health Services remote monitoring programs. The App is not directed at children under 13 years of age, and we do not knowingly collect personal information from children under 13. If you believe a child under 13 has provided us with personal information, please contact us so we can take appropriate action.

11. Third-Party Services

The App does not contain any third-party advertising SDKs, analytics frameworks, social media integrations, or tracking technologies. The only third-party service used is ClickSend for SMS delivery as described in Section 3.

12. Changes to This Privacy Policy

We may update this Privacy Policy from time to time to reflect changes in our practices or applicable laws. If we make material changes, we will update the "Last Updated" date at the top of this page. We encourage you to review this Privacy Policy periodically. Continued use of the App after changes are posted constitutes acceptance of the updated Privacy Policy.

13. Applicable Platforms and Domains

This Privacy Policy applies to:

• Trinity CareView mobile application — available on iOS (Apple App Store) and Android (Google Play Store)
• trinitycareview.com — the CareView support and API platform
• trinityemr.com — the Trinity CareView electronic medical record system used by authorized healthcare providers
